Electronic communication
Terms are subject to change without notice.
- No urgent communications. I understand that the electronic communication tools must not be used for emergencies or urgent health questions.
- Response time not guaranteed. I understand that the clinic cannot guarantee that any electronic message will be read or responded to within any timeframe. I understand that it is my responsibility to follow-up if I have not received a response within a reasonable period of time.
- Role of office staff. I understand that office staff authorized by the clinic to review patient charts may read and respond to any electronic message sent through the system.
- Security and confidentiality. Our clinic uses standard measures to protect your health information. Nevertheless, there is still some risk that your health information could be disclosed to a third party. I understand that the clinic cannot guarantee the security of any electronic communications.
- Email offered by third parties. I understand that some employers and online services may read or keep messages sent using their e-mail system. I agree to minimize the use of such services to protect the privacy of my health information.
- Records. I understand and agree that individual care providers may make decisions about my treatment based on information I provide through email and that this information may form part of my health record.
- Account information. I understand that it is my responsibility to monitor email/text messages received at the indicated accounts and to advise the clinic in writing if any account information changes or should no longer be used by the clinic for email communications with myself. I understand that only these accounts will be used by the clinic for communication to me.
- Withdrawal of consent. I understand that I may decide to stop using electronic communication at any time, at which point I must notify the clinic in writing of this decision. I understand that this consent remains effective unless and until it is withdrawn.
- Withdrawal of services. I understand that individual care providers may stop using electronic communication tools at any time. I understand that I may be notified of this decision in person, in writing, or by electronic communication.
- Malware and viruses. I understand that electronic systems may be used to send viruses and other forms of computer malware. To reduce this risk, we will not open or send email containing attachments. Attachments will be sent within a secure message.
- Disclosure. I understand that electronic communications are subject to the same disclosure conditions as office records, including the duty to report applicable to issues such as child maltreatment.
Missed appointments
Please notify our office as soon as possible if you need to cancel or change your appointment.
If you are running more than 5 minutes late to your appointment, please update our office by phone at 236-304-8002. This will help us keep the day running on time.
If you arrive more than 15 minutes late without notice, your appointment might need to be changed to another day. We may consider this a no-show arrival.
If you are a no show for your appointment, we may charge a rebooking fee. Our current fee is $50 for a follow-up and $100 for a new consult. These fees are subject to change.
Communicating with our office
We are unable to answer any health-related questions before your first visit
Call 9-1-1 for emergencies, such as a sign of a possible stroke/warning stroke (TIA)
Consider calling Healthlink (8-1-1) for non-emergencies (e.g. new non-emergency symptom)
We cannot guarantee that calls or messages will be read or replied to within any set amount of time
Terms for use of electronic communications apply to all electronic communications with our office. You will be asked for your consent to this policy as part of the pre-visit questionnaire. You must agree to this policy to use electronic communication tools with our office. We will still be happy to provide in-person care to you if you do not agree to the policy.
